Player Waivers

  1. CONSENT FOR USE OF PERSONAL INFORMATION

I authorize the Canadian Soccer Association, Ontario Soccer Association, Eastern Ontario District Soccer Association, and Smiths Falls and District Soccer Club to collect and use personal information about my child/ward for the purpose of receiving communications from the Canadian Soccer Association, Ontario Soccer Association, District Association, League and Club.

I understand that I may withdraw such consent related to receiving communications at any time by contacting the OSA Privacy Officer at OSAPrivacyOfficer@soccer.on.ca or by mail to: Attention: OSA Privacy Officer, Ontario Soccer Association, 7601 Martin Grove Road, Vaughan ON L4L 9E4.  The Privacy Officer will advise the implications of such withdrawal.

We do not sell or distribute your personal information to any other third party not listed herein.

 

2. Sanctioned and required by The Smiths Falls & District Soccer Club

In consideration of the acceptance of this application form and the permission to participate as a competitor in the Smiths Falls & District Soccer Club soccer program, I, for myself, my heirs, executors, administrators, successors and assigns, hereby release, waive and forever discharge the Corp. of the Town of Smiths Falls, the Smiths Falls & District Soccer Club, the Upper Canada District School Board, the Catholic District School Board of Eastern Ontario, and all other associations, sanctioning bodies and sponsoring companies, and all their respective agents, officials, servants, contractors, representatives, successors and assigns of and from all claims, demands, damage costs, expenses, actions and causes of action, whether in law or equity, in respect of death, injury, loss or damage to person or property howsoever caused, arising or to arise by reason of my or my child’s participation in the Smiths Falls & District Soccer Club program, whether as a spectator, participant, competitor, or otherwise; whether prior to, during or subsequent to an event and not withstanding that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

I further hereby undertake to hold and save harmless and agree to indemnify all the aforesaid from and against any kind of liability incurred by any or all of them arising as a result of, or in any way connected with my or my child’s participation in the said program.

 

3. Leave No Player Behind

For the second year, the Smiths Falls & District Soccer Club will provide the opportunity to provide financial assistance to players and families who have difficulty paying for soccer registration as part of our new Leave No Player Behind policy.

Again this year, the club will accept donations from our members to help cover these costs.  Any amount will be used to pay for registrations for players in our community.  You can donate via the online registration system in the next section.

4a. Under 18 Agreement

 

ONTARIO SOCCER ASSOCIATION PARTICIPATION AGREEMENT

FOR THOSE UNDER 18 YEARS

 

By signing this document, you will waive certain legal rights, PLEASE READ CAREFULLY.

IN CONSIDERATION of allowing my minor child/ward to participate in the programs, activities and events of The Ontario Soccer Association, I ASSURE TO YOU THAT:

  • I am the parent/guardian of the participant named in the registration form, have full legal responsibility for decisions regarding the above-named participant
  • I believe that my child/ward is physically, emotionally, and mentally able to participate in the programs, activities, and events of The Ontario Soccer Association
  • I hereby acknowledge that I am aware of the risks and hazards associated with or related to soccer. The risks and hazards include, but are not limited to, injuries from:
  • Executing strenuous and demanding physical techniques in soccer
  • Dryland training including weights, running, and massage
  • Grass, turf, and other surfaces including bacterial infections and rashes
  • Falls to the ground due to uneven or irregular terrain or surfaces
  • Collisions with walls and soccer equipment
  • Failure to properly use any piece of equipment or from the mechanical failure of any piece of equipment
  • Extreme weather conditions which may result in heatstroke, sunstroke, or hypothermia
  • Contact, colliding or being struck by other participants, spectators, equipment, or vehicles
  • Vigorous physical exertion and strenuous cardiovascular workouts
  • Exerting and stretching various muscle groups
  • Travel to and from competitive and/or non-competitive events and activities which are an integral part of the organization’s activities

FURTHERMORE, I am aware that my child/ward may:

  • Sustain injuries in soccer that can be severe, cause spinal cord injuries and even be fatal
  • Experience anxiety while challenging himself/herself during the activities, events, and programs
  • Come into close contact with other participants, including the possibility of accidental and unexpected contact
  • Risk of injury is reduced if he/she follows all rules established for participation
  • Risk of injury increase as he/she becomes fatigued

I UNDERSTAND AND AGREE, on behalf of myself, my heirs, assigns, personal representatives and next of kin that my signing/submitting of this document constitutes:

  • I am registering my child/ware willingly and my child/ward is participating voluntarily in these activities, events, and programs
  • I agree that there are risks in soccer as described above and my child/ward will be exposed to these risks and hazards
  • I agree to accept all these risks and hazards and be responsible for any injury or other loss which my child/ward might receive while participating in these events, activities, and programs
  • If something happens to my child/ward, I release the Organizers of responsibility for any claims, demands, actions, and costs which might arise out of my child/ward’s participation.  I understand “Organizers” to mean: The Ontario Soccer Association, District Association, Leagues, Clubs and their directors, officers, members, employees, volunteers, officials, participants, clubs, agents, sponsors, owners/operators of facilities, and representatives

Accident Insurance: Executing this agreement will not preclude you from accident insurance coverage, subject to the terms and conditions of The Ontario Soccer Association’s insurance policy.

 

ACCEPTANCE OF TERMS AND CONDITIONS

In consideration of the acceptance of my child/wards membership in The Ontario Soccer Association, District Association, and Club, I, the parent guardian (for the participant under 18 years of age), agree as follows:

  • I understand that my child/ward cannot play in any sanctioned soccer game until after the registration form has been validated and the registration date has been entered in the OSA computerized registration system
  • I have reviewed the participation agreement above and my signature or submission of this registration form indicates my agreement with such participation agreement
  • I am aware of The Ontario Soccer Association, Eastern Ontario District Soccer Association, Smiths Falls and District Soccer Club, and League bylaws, policies, rules and regulations, and agree to abide by them and to be bound by them
  • I accept sole responsibility for my child/wards personal possessions and athletic equipment
  • I accept all liability for any damage to the playing equipment caused by my child/wards careless, negligent and/or improper handling

 

4b. Over 18 Agreement

ONTARIO SOCCER ASSOCIATION WAIVER AND RELEASE OF LIABILITY

By signing or submitting this form you give up important legal rights. Please read carefully!

This is a binding legal agreement.  As a Participant in the programs, activities and events of the Ontario Soccer Association, their Districts, Leagues and Clubs, the undersigned acknowledges and agrees to the following terms.

The Ontario Soccer Association, their Districts, Leagues and Clubs, directors, officers, members, employees, coaches, volunteers, officials, participants, clubs, agents, sponsors, owners/operators of facilities, and representatives (the Organization) are not responsible for any injury, damage or loss of any kind suffered by a Participant during, or as a result of, any program, activity or event, caused in any manner whatsoever including, but not limited to, the negligence of the Organization.

IN CONSIDERATION of my participation as a Participant in such programs, activities and events, I hereby acknowledge that I am aware of the risks and hazards associated with or related to soccer. The risks and hazards include, but are not limited to, injuries from:

  • Executing strenuous and demanding physical techniques in soccer

  • Exerting and stretching various muscle groups
  • Travel to and from competitive and/or non-competitive events and activities which are an integral part of the organization’s activities

FURTHERMORE, I am aware:

  • That injuries sustained in soccer can be severe, cause spinal cord injuries, and even be fatal;
  • That I may come into close contact with other participants, including the possibility of accidental and unexpected contact;
  • That I may experience anxiety while challenging myself during the activities;
  • That my risk of injury is reduced if I follow all rules adopted during training; and
  • That my risk of injury increases as I become fatigued.

IN CONSIDERATION of the Organization allowing me to participate as a Participant, I agree:

  1. a)     To assume all risks arising out of, associated with or related to my participation;
  2. b)    To be solely responsible for any injury, loss or damage that I might sustain while participating; and
  3. c)     To release the Organization from liability for any and all claims, demands, actions and costs that might arise out of my participating, even though such risks, injuries, loss, damage, claims, demands, actions or costs may have been caused by the negligence of the Organization.

Accident Insurance:

Executing this agreement will not preclude you from accident insurance coverage, subject to the terms and conditions of The Ontario Soccer Association’s insurance policy.

 

ACCEPTANCE OF TERMS AND CONDITIONS

In consideration of the acceptance of my or my child/wards membership in the Ontario Soccer Association, District Association and Club, I, the participant, agree as follows:

  1. I understand that I cannot play in any sanctioned soccer game until after this registration form has been validated and the registration data has been entered in The Ontario Soccer Association computerized registration system.
  2. I have reviewed the waiver attached and my signature affixed hereto indicates my agreement with such waiver.
  3. I am aware of The Ontario Soccer Association, Eastern Ontario District Soccer Association, Smiths Falls and District Soccer Club, and League bylaws, policies, rules and regulations and agree to abide by them and to be bound by them.
  4. I accept sole responsibility for my personal possessions and athletic equipment.
  5. I accept all liability for any damage to the playing equipment caused by me careless, negligent and/or improper handling.

 

 

4c. Supplemental Adult Player Agreement

 

I agree to abide by the fundamental rule that I will not do anything to jeopardize another players safety.  I will abide by the rules of soccer (FIFA) and the Smiths Falls & District Soccer Club.

PLEASE TAKE NOTE Any player acting in an aggressive or overly physical manner will be subject to disciplinary action which may lead to suspension.  Aggressive Conduct is defined as: any physical force that injures or has the potential to cause injuries or abuses any individual or property.  Aggressive Conduct also includes any attempt to make threatening physical contact with another individual.  This also includes any verbally or physically threatening demeanor towards any individual.  Players are to respect each other and the game Officials.  Those not respecting other players and game Officials may face game or season suspension(s) and League fines.

ALL LEAGUE FINES ARE THE RESPONSIBILITY OF THE PLAYER WHO COMMITTED THE OFFENCE AND MUST BE PAID BY THE PLAYER.

Failure to pay fines will result in the Club suspending the player until the fine is paid in full.

No refunds will be given to any players suspended for the season.
